On 27 February 1944, PBY-5, Bureau No. 08354, of Squadron NO. 69, sank at her moorings at Palm Island in four fathoms of water. In the absence of more conclusive evidence, it was believed that the water had entered through the experimental four fixed .50 caliber gun mount in the nose of the aircraft. This reduce dthe operating strength of the Squadron down to 14 aircraft.
