While making a good approach landing at Williamtown airfield in New South Wales on 23 February 1942, P-40 Kittyhawk, #41-5649 piloted by Lt. J.D. Livingstone (Service No. 0-376528) of the 9th Pursuit Squadron, 49th Fighter Group, was thrown on its nose when the landing gear gave way.

The aircraft was repaired and re-issued again, to the RAAF as one of the original A29-1 to 25, in early March 1942.

This aircraft had been involved in an earlier accident at Canberra, ACT in mid February 1942 when it was attached to the 8th Pursuit Squadron.
